Definition

What is androstenedionees?

Androstenedione is used to evaluate the performance of your adrenal glands, ovaries or testes and check the adequacy of male hormone production. This test is also used to ensure the cause of excess male hormones in women. Androstenedione (AD, DHEA, and este of sulfuric, DHEAS), which is produced in the sex glands and adrenal glands, is a pre-substance of testosterone and estron. While pre-substance cortisol is 11-deoxycortisol, 17-hydroxyprogesterone, 17-hydroxypregnenolone, and pregnenolone.

Children with congenital adrenal hyperplasia (CAH) have mutated genes that can inhibit enzymes in the synthesis of cortisol, testosterone, aldosterone and estrogen. This has a large effect on decreasing cortisol and aldosterone production. Low cortisol levels stimulate the pituitary gland to produce ACTH and stimulate the adrenal cortex so that androgen levels increase. In most cases, CAH is a recessive autosom. CAH symptoms include masculinization in female patients caused by excess androgens, secondary salts gradually disappear due to a lack of cortisol and aldosterone, or high blood pressure due to an increase in mineralocorticoids. In addition, CAH also exhibits mild symptoms such as early puberty, acne, excessive hair growth, irregular menstrual cycles and infertility.

Patients who have polycystic ovary syndrome (Stein-Levanthal) have a high amount of androstenedione. Meanwhile, patients with adrenal gland cancer have high DHEAS levels.

When should I undergo androstenedionees?

This test can be done along with other hormone tests, especially when the doctor detects that there is an excess (or even lack of) the amount of androgens in the patient's body. Usually the doctor will also check the function of the adrenal glands, ovaries or testicles. This test can be applied to babies with ambiguous genitalia, or baby girls who have characteristics of masculinity. Alleged causes of this are CAH or diseases caused by excessive androgen levels.

This test can also be done for boys who experience symptoms of early / delayed puberty, enlargement of penis and muscle size, and development of pubic hair at an early age.

Prevention & warning

What should I know before undergoing androstenedionees?

Some drugs such as clomiphene, levonorgestrel, corticotrophin, and metyrapone can increase the concentration of androstenedione, while corticosteroid drugs such as dexamethasone may have the opposite effect.

If you are undergoing a radioimmunoassay method, radiation imaging carried out in the previous week can affect the test results.

Process

What should I do before undergoing androstenedionees?

Especially for women, the most effective time for a blood test is one week before menstruation. You are recommended to use short-sleeved clothing to make it easier to take blood samples from your hands.

What is the process of androstenedionees?

The medical personnel who are in charge of taking your blood will take the following steps:

  • wrap an elastic belt around your upper arm to stop the blood flow. This makes the blood vessels under the bond enlarge making it easier to inject needles into the vessels
  • clean the part to be injected with alcohol
  • inject a needle into a vein. More than one needle may be needed.
  • attach the tube to the syringe to fill it with blood
  • remove the ties from your arms when taking blood is enough
  • attach gauze or cotton to the injected part, after the injection is finished
  • put pressure on the part and then put on a bandage

What should I do after undergoing androstenedionees?

Some people may feel pain when a syringe is inserted into the skin. But for most people, the pain will gradually disappear when the needle is right in the vein. Generally, the level of pain experienced depends on the expertise of the nurse, the condition of the arteries, and a person's sensitivity to pain.
After going through the process of taking blood, wrap your hands in a bandage. Press the vein slowly to stop bleeding. After doing the test, you can do activities as usual.

Explanation of Test Results

What do the test results mean?

Normal:

ManWomen
AD0.6 - 2.7 ng / mL0.5 - 2.7 ng / mL
DHEA1.0 - 9.5 ng / mL0.4 - 3.7 ng / mL
DHEA S280 - 640 mcg / dL65 - 280 mcg / dL

Abnormal:

Index goes up:

  • adrenal gland tumor
  • congenital adrenal hyperplasia
  • Ectopic tumors release
  • Cushing's syndrome (in some cases)
  • Stein - Leventhal's syndrome
  • arantius ligament tumor

Down index:

  • damage to the sex glands
  • disorders of primary or secondary adrenal glands

If the adrenal test is normal, it means that your adrenal glands work well. This is supported by normal levels of androstenedione and androgens. However, the concentration of androstenedione can increase (even remain normal) if there is an adrenal tumor or cancer. This depends on the amount of hormone released.

The increase in androstenedione levels is directly proportional to the production of adrenals, testicles, or ovaries. This can cause tumors in the adrenal area, cancer, or congenital adrenal hyperplasia. If this test has not provided a specific diagnosis, the doctor will give the next test.

Low levels of androstenedione can be caused by adrenal dysfunction, adrenal damage, or damage to the testicles and ovaries.

The normal range for androstenedionees tests can vary depending on the laboratory you choose. Discuss with your doctor if there are questions about your test results.
