What is the anti-glomerular basement membrane?

Anti glomerular basement membrane tests are used to diagnose Goodpasture and nephritis syndrome. Goodpasture syndrome is an immune disorder that causes antibodies to attack the glomerular membrane basement and pulmonary alveoli. These antibodies cause the immune system to damage the kidneys and lungs. Patients often experience 3 symptoms at the same time: glomerulonephritis, pulmonary hemorrhage and the appearance of anti-glomerular basement membrane antibodies. Approximately 60% - 75% of immune patients with glomerulonephritis experience lung complications.

When do I have to undergo an anti-glomerular basement membrane?

The Anti-GBM test is indicated if doctors find symptoms of Goodpasture and lupus nephritis syndrome. The following are symptoms of both diseases:

  • weight loss
  • shivering fever
  • hemoptition
  • nausea and vomiting
  • chest pain
  • anemia is caused by bleeding in the lungs and kidneys
  • weakening of the lungs (breathing difficulties)
  • kidney disorders

Prevention & warning

What should I know before undergoing an anti-glomerular basement membrane?

Your doctor may recommend a lung or kidney biopsy to observe these antibodies using an immunohistochemical technique. Another method is to use a more rapid and reliable serology in the diagnosis of Goodpasture syndrome, especially if the biopsy in a patient is difficult to implement. In particular, antibody serum can help monitor if the patient's response is positive for treatment.

It is important that you understand the warnings above before running this test. If you have questions, consult your doctor for further information and instructions.

Process

What should I do before undergoing an anti-glomerular basement membrane?

Follow the doctor's explanation and instructions regarding the test, you are required to fast for 8 hours before the test is done. You are allowed to drink water during fasting.

If you have a lung or kidney biopsy to take tissue samples, you should pay attention to the doctor's explanation of the biopsy procedure.

You are recommended to wear clothes with short sleeves to facilitate the process of taking blood.

What is the basement membrane anti-glomerular process?

The medical personnel who are in charge of taking your blood will take the following steps:

  • wrap an elastic belt around your upper arm to stop the blood flow. This makes the blood vessels under the bond enlarge making it easier to inject needles into the vessels
  • clean the part to be injected with alcohol
  • inject a needle into a vein. More than one needle may be needed.
  • attach the tube to the syringe to fill it with blood
  • remove the ties from your arms when taking blood is enough
  • attach gauze or cotton to the injected part, after the injection is finished
  • put pressure on the part and then put on a bandage

What should I do after undergoing an anti-glomerular basement membrane?

The doctor or nurse will take your blood sample. Pain depends on the skills of the nurse, the condition of the blood vessels and your sensitivity to pain.

After the blood collection process, you are recommended to bandage with a bandage and lightly press your blood vessels to stop bleeding. You can move as usual after undergoing a test.

If you have questions about the process of this test, consult your doctor to understand further.

Explanation of Test Results

What do the test results mean?

Normal results:

Network:

Negative: no spots of immunofluorescence (IF) are found in the cell membranes of the kidneys and lungs.

Blood: with enzyme immunoassay (EIA)

  • negative: <20 units
  • fluctuation (borderline): 20-100 units

Abnormal results:

Positive: blood (EIA)> 100 units

  • Goodpasture syndrome
  • autoimmune glomerulonephritis
  • lupus nephritis

The basement membrane anti-glomerular test results can vary depending on the laboratory. Consult your doctor if you have questions about the test results.
