To become a member of the BPJS, you need to provide personal data information to BPJS. So before registering, prepare the data file first.

1. Prepare a personal data file or family data

Some of the data you need to register as a BPJS member, include:

• KTP, SIM, or passport (photocopy or results scanning)
• Latest photo size 3 × 4 cm per sheet per person (already printed or shaped file in format .jpg)
• Birth certificate (photocopy or results scanning)
• Marriage certificate (photocopy or scanning) for those who are married
• NPWP card and photocopy of passbook

Then, prepare additional data according to your work and profession, for example:

• Decree of Establishment as a State official legalized for state officials
• Latest PNS SK for civil servants
• Final Decree of Decree for TNI and POLRI
• Proof of self as labor and employment agreement for private employees
• Legalized salary list
• KP4 letter, which is a certificate of getting a family allowance payment
