Penile injury can occur intentionally or unintentionally. Intentional penile injury is usually the result of a fight or violence. Types of penile trauma are considered as urological emergencies and usually require surgical intervention. The goal of treatment for penile injury is to maintain the penis, restore erectile function, and the ability to excrete urine when erect. That's because penile trauma may also involve the urethra (the tube in the penis used to urinate and ejaculate).

Understand how the penis works normally

Before we get into the types of penile injuries, it's good to know how the penis works first. The two main functions of the penis are urination and reproduction. Inside the penis there are three tubes, one of which is called the urethra. The urethra is hollow and allows urine to flow from the bladder through the penis and out. The other two tubes are called the corpora cavernosa, which is a soft sponge tube that will eventually be filled with blood during erection. The three tubes are wrapped together by a very strong fibrous sheath, called tunica albuginea.

During sexual activity, penile erection allows the penis to be inserted into the woman's vagina. In this situation, the urethra acts as a conduit for semen that will ejaculate into the vagina, which allows fertilization and pregnancy.

Types of penile injuries

1. Fracture of the penis (penis is broken)

Penile fractures are tears from the corpora cavernosa. Penile tears are relatively rare, but this is considered a urological emergency. Sudden twisting of the penis in an erect state can tear the tunica albuginea, so that the penis is broken. One or both corpora may be involved, and injury together with the urethra can also occur. Urethral trauma is more common when the two corpora cavernosa are injured.

Broken penises can usually be diagnosed only based on history and physical examination findings. However, in vague cases, diagnostic cavernosography or MRI must be performed. In addition, simultaneous urethral injury must also be considered, so that a retrograde urethrographic study before surgery must be performed.

2. Amputation / cutting of the penis

This is when a part or all of the penis is cut. Usually associated with incidents due to anger, jealousy, or psychiatric disorders. Acute blood loss due to the tragedy of cutting the penis may be quite large and life threatening, especially if amputation occurs when the penis is erect. Operations must be carried out immediately to ensure that the cut part can still "live".

The goal of surgery is to restore penis length and penis function, if possible. Because the nerves to the erectile tissue are usually not damaged, the penis that has been cut is usually still erect. Micro surgery (surgery performed by the surgeon by looking under the microscope) is needed to restore any level of sensitivity.

Compared to other forms of reconstruction, micro surgery offers the best opportunity to have a functioning urethra. Special care must be taken to reconnect blood vessels, especially the inner back blood vessels, in order to restore the venous canal, and prevent swelling and disruption of blood flow after surgery.

3. Translucent wounds

This injury is the result of being hit by a ballistic weapon, shrapnel, or puncture to the penis. Penetrating wounds occur most often in war conflicts and are less common in the common people. Penetrating wounds can involve one or both of the corpora, urethra, or soft tissue of the penis.

4. Penile soft tissue injury

Soft tissue injury to the penis can be caused by several factors, such as infection, burns, human or animal bites, and injuries involving machinery. In this case, the corpora is not involved.

How to avoid penis injury?

The above penis trauma associated with sexual intercourse, such as a broken penis, can be prevented in most cases. Penile fractures often occur when the position of a female partner is above. If your erect penis accidentally slips from your partner's vagina, stop immediately, before the penis is crushed by your partner's body which can cause the penis to break. For other trauma, be careful when doing work, especially if you are near a machine, sharp weapons, and so on.
